A communion prayer

This afternoon/night I remembered a communion prayer, which I think I was thought around the time of my first communion, when I was 10 years old.

I has been a while since I have stopped praying, perhaps since I was 18 years old.

I did not found the prayer online, but calling from memory this is the closest version. If someone also knowing, help me editing.

I will write in Spanish as I was taught, and what I remember:

Señor, yo creo firmemente
que estas aquí presente
en cuerpo y sangre
en la hostia consagrada.

Tú que fuiste crucificado
por culpa de mis pecados,
al tercer día resucitaste
gloriosamente.

Yo vengo a darte las gracias,
porque sé que estás en mí.
Sé que eres mi amigo
Y que nunca me abandonarás
Hasta el fin de mis días.
